New Patient Enrollment
Welcome to Phillips Pediatrics! Whether you are expecting a new child, have recently moved to the Oxford area, or are looking for a new medical home for your children, we thank you for trusting us with your child’s health. We accept new patients from infancy through the teen years.
Are We a Good Fit?
Finding a pediatrician who is a good fit for your family is key to a positive and successful relationship. Phillips Pediatrics strives to keep your children as healthy as possible from birth through college. An essential part of managing your child’s health includes ensuring they are up-to-date with routine checkups and vaccines. We follow the American Academy of Pediatrics Bright Futures Guidelines to provide the highest standard of care. Phillips Pediatrics requires non-vaccinated or under vaccinated children to be on a catch-up schedule.
Moving to the area
or transferring from another practice?
If you are a new patient transferring to our practice or moving to the area, this is your first step towards joining the Phillips Pediatrics family. Please fill out this new patient application form.
Once your new patient application has been successfully submitted, PLEASE ALLOW 3 BUSINESS DAYS FOR A RESPONSE. If you are notified that we have the availability for your child to become a patient at this time, you will receive a link for the remaining registration forms.
Scheduling is not allowed until we have your child’s vaccine record and the enrollment process is fully completed.
Newborn Enrollment
Congratulations on your baby! Please fill out our newborn forms once your baby is born. You have 30 days after birth to file for insurance. Once your baby turns 1 month, we require insurance information or a self-pay plan. The following forms must be completed prior to your first newborn visit:
- Online Family Registration (Required for Each Household)
- Online Medical History Questionnaire (Required for Each Child)
Expecting?
Having a baby is one of the most rewarding experiences in your life. At Phillips Pediatrics, we are committed to keeping your baby healthy.
Our pediatric staff will guide you through milestones and give you the confidence and reassurance you need as you transition to life at home with your little one.
We love seeing families grow and encourage you to meet our team. We offer complimentary prenatal visits. This also allows you to meet us and make sure we are a good fit for your family.

Scheduling Baby’s First Visit
We are happily accepting new infants to the practice. Your baby’s first examination at the hospital will be with a pediatric hospitalist or on-call pediatrician. We will see your baby 2-3 days after discharge and regularly after that for checkups.
If you are adopting a baby or being discharged from the NICU, we like to speak with the discharging hospital to discuss any medical concerns and ensure timely follow-up.
Upon discharge, either you or the nursery will need to schedule a newborn visit with our office. Please fill out our newborn enrollment forms and prior to your first visit. You have 30 days after birth to file for insurance. Once your baby turns 1 month, we require insurance information or a self-pay plan.
We look forward to seeing your little one!
Lactation Specialist
Our staff has both personal experience and training in breastfeeding medicine. As Certified Lactation Counselors, we incorporate lactation counseling into newborn visits and give additional support if there are problems or concerns. While we advocate breastfeeding to optimize health, we are here to guide and support every family’s unique feeding journey.
